Divine Companion Care is a non-medical home care company dedicated to providing the best care to our clients at their most comfortable environment, their home. Started as a PCA then moved up to HHA where I worked as an HHA for many years. Throughout the years I have worked in Assisting Living, also in a Hospital for over four years.
The main goal of hospice is to see that their patients are given the respect and dignity that they deserve in their stage of life. Despite what many have come to believe, hospice care does not mean “giving up” or “choosing death”— it means choosing to live the best life possible when reaching the end of your days. We offer a full range of homemaker, private-duty nursing, and medication management services. Our homemaker services department is tailored to provide plans that give our seniors and other adult clients the right amount and the right type of help to support independent living at home. Chicago home care services are provided by special caregivers who are trained and compassionate to promote independent living to its fullest. The Divine Compassion Hospice team includes a doctor, nurse, social worker, counselor, chaplain, home health aide, and trained volunteers.

Volunteers are the backbone of hospice care, working in tandem with healthcare professionals to provide the best experience possible for patients and their loved ones. They complete numerous different tasks to keep our services efficient, such as preparing meals, doing simple chores, and running errands that a patient would be unable to do otherwise. Hospice offers numerous programs to ensure that these volunteers are well-prepared and reliable. Hospice care is a specialized form of care that offers support and comfort for people who have been diagnosed with advanced terminal illness. An experienced medical team and trained volunteers work together to ensure that their needs are fulfilled. Depending on the patient, it can be provided at a nursing home, a long-term care facility, an inpatient hospital, or even home.
Patient's regular physician often provides care, in cooperation with a hospice medical director. Our caregivers are supervised by Registered Nurses, Licensed Practical Nurses, and Certified Nursing Assistants. As a person gets older, sometimes even routine daily tasks can become a challenge. We offer a full range of homemaking, medication management, and personal care services to ensure that your loved one remains safe and happy at home. In general, when a person’s illness reaches a point where they can no longer be controlled or cured, they will be able to find solace in hospice care. Divine Home Health Care works with compassion, patience and respect.
We can provide tube feeding, wound care, and even colostomy care all right at home. In this common form of hospice, patients choose to receive care in the comfort of their own home. Remaining in a familiar environment, family members are still able to look after them with the assistance of visiting healthcare professionals.

Our caregivers can help prepare light meals and ensure that your loved one is not toiling to prepare daily meals. You don’t want to have to second guess your health management after a surgery or any hospitalization. Our caregivers are supervised by Registered Nurses , Licensed Practical Nurses , and Certified Nursing Assistants . Patient is assigned a case manager nurse, who typically visits one to three days a week. Hospice patients and their caregivers can also access an on-call nurse 24 hours per day. If your loved one cannot drive, our caregivers are available to help do their errands including grocery shopping. If you need help running errands yourself, our caregivers are available to provide companion.
Companionship is key to a trusted relationship with our caregivers. We not only strive to help you with everyday tasks but want to develop a caring relationship with you. We provide one-on-one attention and care that cannot compare in other settings.
